Prestige Healthcare Group: Salesforce Foundation for a Multi-Service Home Care Operation

Prestige Healthcare Group needed a Salesforce platform that could hold intake, CDPAP applicant processing, and ongoing caregiver management in one place. Madison Ave scoped, built, and migrated the data to make it happen.

Prestige Healthcare Group runs a home care operation with multiple service lines, including CDPAP (Consumer Directed Personal Assistance Program), a New York-specific model where the consumer chooses and directs their own caregiver. Running this at scale means tracking both consumers and caregivers as distinct populations, with overlapping compliance requirements.

What Madison Ave Delivered

  • Scope definition. Mapped the Prestige service model into a clean Salesforce object model before building, so CDPAP applicants and standard home care cases shared infrastructure without collapsing into each other.
  • Data migration. Structured legacy data exports and upserts into Salesforce components, preserving history while cleaning inconsistencies.
  • Unique external ID architecture. Handled the external ID strategy properly so integrations and imports stayed reliable long-term.
  • CDPAP applicant workflow. Built the applicant scope as its own workflow with the right stage gates for compliance.

Why It Worked

CDPAP is the kind of workflow that a generalist SI gets wrong. The regulatory requirements are specific (New York, Medicaid), the consumer-caregiver relationship is unusual, and the audit trail has to be airtight. Madison Ave had done this work before and delivered a platform Prestige could operate on without rework six months in.
