Anchor Health Puts Agentforce to Work in the Field

Six months after standing up Pulse Platform, Anchor Health's next frontier was Agentforce: bringing AI agents into the field sales motion for caregiver recruiting, patient intake, and compliance. Same team. Same white-glove delivery. New platform, new ceiling.

Anchor Health’s six-month Pulse implementation was the foundation. It unified recruiting, intake, onboarding, and case management on Salesforce and gave the business a single source of truth. What came next was the next logical move: put Agentforce on top of that clean foundation and let AI agents do the work the team shouldn’t have to.

Why Agentforce

Field sales in home health is mostly relationship work, but it’s also mostly paperwork and follow-up. Referral sources to track. Caregivers to schedule. Compliance checkpoints that move every week with state regulation. Anchor’s team was doing this work well, but they were spending energy on the wrong parts of it. Agentforce offered a way to shift the mechanical work to AI and give the humans back their time.

What We Built

  • Caregiver matching agents. When a new patient intake comes in, an Agentforce agent evaluates clinical need against the active caregiver pool, factoring certifications, geography, language match, and availability. Surfaces the top three matches to the intake coordinator in seconds.
  • Referral follow-up agents. Field reps no longer hand-craft follow-ups after referral source visits. The agent drafts contextual outreach based on the conversation notes and the referral source’s history, ready for one-click send.
  • Compliance checkpoint agents. State-specific compliance rules (NY, PA, MI) are codified. The agent monitors active cases and flags upcoming renewals, documentation gaps, or regulatory changes before they become findings.

Why It Worked

Agentforce is only as good as the data underneath it. Pulse had already unified Anchor’s data model across intake, caregiver, compliance, and EMR integrations. That meant the agents had real context, not a siloed snapshot. Agent outputs were accurate the first week, not the fourth month.

The Outcome

Intake coordinators spend less time matching and more time with families. Field reps close the loop on referrals without context-switching to a drafting tool. Compliance gaps surface days earlier. And Anchor is now one of the first launch-stage ISV adopters of Agentforce in home health, a position that’s already opened conversations with peer operators.
